    Key Responsibilites:

    • Review applications from AIC. Interview client’s and caregivers during enrolment into our programme/Home. Conducting Biopsychosocial assessment to assessing the strength and resilience of the client, families, and social support systems.
    • Supports Medical Social Worker / Senior Medical Social Worker during resident’s admission in preparation for admission documents.
    • Build rapport with residents/clients and their families. Maintain constant communication with clients while addressing their concerns and goals and helping them keep positive attitudes. And educating the family members on the physical and psychosocial needs of the clients and ways they can access internal and external resources.
    • Provide casework management for our clients and coordination with external agencies such as hospitals and polyclinic for clients who require financial assistance for their medical appointments.
    • Works with Community and Partnerships and Allied Health to develop initiatives for client/family engagement
    • Develop and implement individual care plans (ICP) for clients and provide therapeutic support as needed. – working with other multidisciplinary team (MDT) members.
    • Execute the ICP and monitor progress. Develop and implement programmes and activities through a planned/structured manner that is aligned to SASCO values to meet the clients care needs.
    • Coordinate with PT and OT departments to assist in helping clients feel comfortable and confident.
    • Put up 6th monthly ICP review reports and/or 3rd monthly review records for clients.
    • Evaluate existing, new and prospective clients based on their needs, desires and limitations.
    • Update clients particulars and social report/review. Conduct casework evaluation, maintain case notes/documents.
    • Person centred counselling for clients and/or families - counselling clients who suffer from poor mental health states (e.g. depression, anxiety), and coping and adjustment difficulties in their transition.
    • Risk assessment - assessing risk of self-harm (e.g. suicide) and to others (e.g. family violence, elder abuse).
    • Financial assessment and fund management and prepare periodic reports as required - identifying and referring cases for financial assistance (e.g. generate Means Test Subsidy Change Rate Report from MOH for invoices printing/Changed Subsidy report, IHDC Master
    • Following up and checking on accounts receivable status Identify cases to put up for Medifund/financial assistance scheme
    • Any other additional tasks assigned by the Management.

    Qualifications & Skills Required:

    • Diploma in Social work
    • At least 2 years’ social work experience
    • Prior experience in residential setting for the elderly
    • Have good knowledge of government financial schemes
    • Ability to work with different groups, elderly, stake-holders
    • Excellent written and inter-personal communication skills

Company Profile

SASCO Senior Citizens’ Home was set up on 21 January 1981 at the void deck of a HDB block in Telok Blangah to provide lodging and care for the destitute elderly regardless of race, language or religion. We were registered with the Commissioner of Charities in the same year.



We are also affiliated to the National Council of Social Service (NCSS) and a member of the NCSS Charitable Fund. When it was first set up, the Home took in 10 residents from the now defunct Woodstock Home. The intake soon expanded, with the Home in Telok Blangah consistently running at the full capacity of 20 residents. SASCO Home’s Sheltered Home facility has since moved to Hong San (Evergreen Place), where we are able to accommodate up to 102 seniors, while the Telok Blangah facility has been converted to a Day Care Centre.



Besides lodging and personal care, the Sheltered Home provides a range of organised activities for our residents. These include karaoke sessions, board games, art and craft, and other group activities. To meet the residents’ rehabilitative needs, professional physiotherapy sessions are conducted at least twice weekly. Spiritual needs are also not forgotten, and there is a private prayer room for residents of all religions to use. Basic recreational resources such as television sets, reading materials and board games are also available.
