  Staff Nurse for Home base Patient Care

  
  Home Nursing Care Plan: Assess client’s health, social history, and home environment; advise on care plan; document and upload relevant information.

  Nursing Procedures: Perform wound care, manage catheters and feeding tubes, and provide other clinical care as per standards.

  Health Education: Provide customized training for caregivers to improve care.

  Administrative Duties: Cover outpatient clinics, triage referrals, manage healthcare financial schemes, and handle fees and receipts.

Desired Candidate Profile

• Job Requirements:

  • Registered with Singapore Nursing Board with valid practicing certification
  • Minimum of 3 years’ nursing experience working in a Medical OR Geriatric ward or in community setting within the past 3 years
  • Experience in Oncology or Palliative Care will be an advantage
  • Independent and highly reliable, who consistently performs well in all aspects of work
    
  • Able to make good clinical judgements regarding patient care in a home-care setting
  • Has keen interest in supporting End of Life (EOL) care for homebound seniors
  • Willing to support After Office Hour (AOH) care for EOL patients on rotational basis
